  1. Student’s CNIC or B-form – Copy of both parents’ CNICs * [Copy of guardian’s CNIC in case of death] * Attested Copy of Salary Slip *(Latest Pay Slip of the Year 2022) of In-Service employees / Retired employees (Female or Male) can use an attested copy of pension book (PPO) Copy of Student’s Result card for the Year 2022 (with at least 60% or higher marks *Proof) from Board/University (*Students are ineligible if any subject is failed or absent) and Copy of downloaded result card from the internet is not acceptable.
  2. Add the Ledger Number to the application form and a copy of the Sanction Letter if the grant is paid out every month.
  3. If the student or child goes to a private school, the certificate of recognition from the board or university and the registration number for 2022-23.
